As a new patient, we want you to be as prepared for your first appointment as possible. We have listed some
general information for you to know for that appointment. If you have questions at any time, please contact our

What to bring:
Your insurance card
A picture ID, if available
Your referral notice or letter, if applicable
A check, cash, credit card for co-payments, deductible, and co-insurance amounts due before you visit
Any lab test, medical records, X-rays, or the like that is pertinent to your current medical care that will help our medical provider.
When to arrive:
Please arrive in our office 20-25 minutes early, as we will need you to verify your address and insurance
information. This will allow our patient service staff to complete the preparation of your medical forms and
records as necessary.
Forms to Complete Prior to Your First Visit:
This is very important. You can either download or we can mail you several forms
to read, complete and bring with you at the time of your visit.
